In Home Health, PT/PTA help patients maintain mobility, manage pain, and ensure safety, The primary goal for is to enhance the patient's quality of life, focusing on full rehabilitation.
Home Health OT'S focus on maximizing functional independence in daily activities and supporting both the patient and caregivers.
Home Health nurses collaborate with the physician, home health aides, social workers, and rehab team to ensure that the individual and their loved ones have the support they need.
A Nurse Practitioner (NP) uses their advanced medical skills to provide specialized focused care for patients . They act as a bridge between a regular doctor and the home health team.
Home Health speech therapy assist patients in finding new ways to communicate, manage swallowing problems, and maintain cognitive function, working with families and caregivers to implement effective strategies and support. The goal is to ensure patients can express their needs, enjoy meals, and remain connected to loved ones.
CHHA helps individuals with daily activities, providing personal care such as bathing, dressing, and grooming, and assisting with household tasks like meal preparation and light housekeeping. and providing companionship to clients who are elderly, disabled, or recovering from an illness, helping them maintain independence at home
MSW asesses a patients' and families' social, emotional, and environmental needs, offering counseling and education about health conditions, and connecting them to community resources and financial assistance. They also help with care coordination, develop individualized care plans, and advocate for patients to navigate the healthcare system

To qualify for Medicare home health services, a patient must be confined to the home and be under physician care who is a doctor of medicine, a doctor of osteopathy, or a doctor of podiatric medicine, and enrolled in the Medicare Program

Hospice care can be provided in a person's home, a dedicated hospice facility, and the goal is to help the person live their remaining time with dignity and as peacefully as possible.

The foundation of our service is a team of highly skilled and trained professionals providing compassionate and personalized care tailored to the unique needs of each individual.
We have a disciplinary team of various professionals, including physicians, nurses, social workers, chaplains, and hospice aides, to provide comprehensive care and support to patients and their families. Other disciplines like physical, occupational, and speech therapists, as well as volunteers and bereavement specialists, may also be involved, depending on the patient's individualized specific needs and the hospice provider's orders.
Hospice offers care from a team of different types of practioners who work together to meet the medical, psychological and spiritual needs of you and your family.
Hospice provides you a phone number to call 24 hours a day, 7 days a week, to help answer any questions you may have.
Once you are admitted into the Hospice Program, you will be provided with:
Your personal physician continues to provide your care while you're in the Hospice Program. A hospice physician works closely with your personal physician and can consult with you.
A nurse visits during the week to instruct your caregiver and to coordinate your care. A nurse informs the hospice physician of your condition on a regular basis.
A social worker helps you and your caregiver by:
A hospice aide visits during the week as needed to help you and your caregiver with personal care such as changing linens, giving you a bath and helping you get dressed.
A chaplain is a minister or counselor dedicated to meeting the spiritual needs of you and your family, no matter your religion. You and your family can ask for the chaplain anytime.
A volunteer is someone from the community who has been trained by our Hospice Program to help you and your family. Volunteers provide services under the direction of a hospice staff member.
Your personal physician is in charge of your care, even after you're admitted to our Hospice Program. A hospice physician will work with your personal physician to provide the care your need.
At some point, you may become confined to bed and unable to turn over or eat without help. Your caregiver may need to change your diapers and linens and care for your skin to prevent bedsores. Hospice nurses will teach caregivers how to perform these tasks.
We understand that caregivers may face difficult situations. A 24-hour phone line puts the caregiver in touch with a hospice-trained nurse who can give advice over the phone or make a home visit if necessary.
